Please select the appropriate package and complete all of the forms. Be sure to print off all forms. Bring all documents with you the first day of employment. These papers must be completed upon arrival for orientation. If you do not have access to a printer, you may stop by Human Resources located in Garrison Tower (directly across NE 13th street from the VA) on the 2nd floor, suite 2200, and use one of our computers OR you may pick up a packet from the front desk during normal duty hours, 8:00 am to 4:30 pm Monday through Friday.
Remember to bring two forms of ID. Your driver's license will be required along with your car tag number and insurance verification form for your car decals. Decals will be issued the first day of orientation. If you have any questions or problems filling out the forms, please call (405) 456-5127.

The Oklahoma City (OKC) VA Medical Center (VAMC), and the University of Oklahoma, College of Nursing (OUCN) established an academic partnership in 2007 known as the VA Nursing Academy (VANA). This partnership was funded by the VA Office of Academic Affiliations (OAA). The partnership was initiated to facilitate stronger and mutually beneficial partnerships between VA’s and Schools of Nursing (SON) and is now known as the VA Nursing Academic Partnership (VANAP) program.
