Veteran Health Information Exchange (VLER)

Veteran Health Information Exchange/Virtual Lifetime Electronic Record (VLER) Health Program

The VLER Health Information Exchange Program helps you “Connect Your Docs” by sharing certain parts of your Veteran health record between the Department of Veterans Affairs (VA), Department of Defense (DoD), and participating non-VA healthcare providers over a secure and confidential network known as the eHealth Exchange. 

Many Veterans receive a portion of their care from non-VA healthcare providers.  Sharing certain parts of your Electronic Health Record (EHR) will help reduce the need for duplicate testing, improve care coordination, as well as allow your VA and non-VA healthcare providers to make more informed decisions regarding your care.

How do I join?

Signing up is fast and easy. You may sign up in person, through the mail, by fax, or online.  Check out the VLER Health Fact Sheet

Online: Simply visit www.va.gov/VLER for a complete list of instructions on how to sign up online.  Please do NOT email completed forms back to the Oklahoma City VA Health Care System.

By Mail:
1. Print and complete the VA Authorization (VA Form 10-0485).  
2. Print and complete the Verification Form.
3. Mail the Authorization form and Verification form to Oklahoma City VA Health Care System, Attn: Release of Information Office (136D), 921 N.E. 13th Street, Oklahoma City, OK, 73104-5028 3

By Fax:
1. Print and complete the VA Authorization (VA Form 10-0485).
2. Print and complete the Verification Form.  
3. Fax the Authorization form and Verification form to the Oklahoma City VA Health Care System's Release of Information Office at (405) 456-1737.

In Person:
1. Print and complete the VA Authorization (VA Form 10-0485).  
2. Print and complete the Verification Form.
3. Return completed forms to the front desk person in the Release of Information Office (ROI) located in Room GB-115 on the ground floor near the North Entrance.
