Discharge Procedures

Discharge Plan

Our goal is to provide you with the best care we can both during and after your hospitalization.  We want you to be informed of what must happen before your discharged.

Early during your stay, your health care team will talk to you about your after hospitalization care needs, anticipated discharge date, and how to receive your follow-up care.

Your health care team works together with a case manager and social worker to help you with any needs you may have when you go home. Some things that happen before you go home are:

- An order for discharge will be written by your doctor.
- Any last lab and other tests will be ordered.
- Other doctor(s) involved in your care may need to see you prior to your discharge.
- Referrals will be made for any home equipment or home care you may need.
- Treatments will be completed.
- Your nurse will prepare discharge instructions.
- Education/instructions will be given to you and/or family.
- Your clothing will be available on the day of your discharge.
- Discharge travel arrangements will be made prior to your date of discharge.

If supplies are needed at time of discharge, the discharge nurse will review them with you. If medication is prescribed, it can be picked up at the outpatient pharmacy as you leave the hospital.

If you are on a special diet, a dietitian will counsel you and your family on how to follow your diet at home.
